Scrip Order Form

Order form in Microsoft Excel Format: (To download form right-click Excel button and select "Save Link As.." or "Save Target As..")

Please use the Scrip Order form for making your Scrip purchases for the 2008/2009 school year. There is a mandatory $200 in Scrip profit obligation this year so please begin to purchase your Scrip as needed.

The form should be completed and then e-mailed to sharhat@pacbell.net. When the form is received, confirmation e-mail will be sent along with instructions on when and how your scrip can be picked up and your payment can be made.

If you would prefer, the form can be taken to the school office during the summer time and possibly the order can be filled with inventory on hand. School hours during the summer will depend upon Charlotte's schedule so please call the school office ahead of time (818-889-9483) to determine if she will be available.

Dennis Uniform Scrip is available during the summer as well. It is also available in the school office. This scrip will also be available on "Orientation Day" but there will be a limited amount of scrip available and it will be on a first come first serve basis. If you plan on using Dennis Uniform for your Scrip obligation, please send in the form ahead of time and the Scrip will be reserved for you on "Orientation Day."

Please note that if Dennis Uniform Scrip is not ordered ahead of time and you plan on purchasing it at the school sale it may or may not be available depending upon the inventory that I will have on hand.
